The following is required for your Immigration Medical Examination:
- – Immunization records (past and current; including COVID-19) –
– Prior I-693 forms cannot be accepted.
– Lab and/or x-ray results for past positive TB test.
– Proof of treatment for active or latent TB.
– I-693 Forms; Name and A–number (if any) must be filled in at the top of all pages. - – The fee for this examination is $275.00. There are often additional fees for required lab work and/or vaccinations.
- **PAYMENT IS DUE OVER THE PHONE BEFORE A SCHEDULED APPOINTMENT IS CONFIRMED!!**

- – Non-English-speaking patients are required to bring their own interpreter to this appointment. Interpreter must be able to speak, read and write in both the patient’s native language and English. CNOS cannot provide an interpreter. Telephonic/video interpreter will not be accepted.
- – Government issued photo ID for patients ages 14 & older (passport &/or driver’s license)
- – Patients under 14 will need to provide a birth certificate or proper affidavits.
- – Parent and/or legal guardian must attend for all patients under the age of 18.
*Disclaimer: If you have received a live immunization, such as MMR and/or varicella, within 30 days of your immigration medical exam appointment, your appointment will need to be rescheduled. Unless otherwise instructed by your provider, we recommend that you wait to receive immunizations until after your immigration medical exam.*
